Job summary

Evolve Therapy Services in North Platte, NE seeks a licensed Speech Language Pathologist for an in-house Skilled Nursing Facility on a PRN basis. You will provide therapy services, develop treatment plans, and ensure CMS-compliant documentation.

Qualifications

• Master's degree in Speech Language Pathology is required.
• Certificate of Clinical Competency (CCC).
• Must have active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with targets, and short- and long-term goals.
• Instructs patient, family, and staff to transfer skills to daily activities.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversight and implementation of FEES/MBS protocols with site coordination.
• Participate in screening protocols to identify resident needs promptly.

Education

Master's degree in Speech Language Pathology

Certificate of Clinical Competency

Active SLP license in state(s) of practice
