Job summary

Evolve Therapy Services, LLC in Nebraska seeks a Speech Language Pathologist for an in-house Skilled Nursing Facility, PRN. You will plan, implement, and direct SLP services to ensure the highest quality resident care.

Requirements include a Master’s in Speech Language Pathology, CCC, and an active state license; ongoing CEU participation is expected. Familiarity with NetHealth EMR is a plus.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency (CCC).
• Active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target lists and goals.
• Instruct patient, family, and staff in speech-language procedures for carryover.
• Complete compliant documentation per CMS expectations in NetHealth EMR.
