Evolve Therapy Services, LLC is hiring a Speech Language Pathologist for an in-house Skilled Nursing Facility in Omaha, NE. This part-time position requires a Master’s degree in Speech Language Pathology, active SLP licensure, and the Certificate of Clinical Competency.

Qualifications

• Master's degree in Speech Language Pathology required.
• Certificate of Clinical Competency required.
• Active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with targets, short-term and long-term goals.
• Instruct patient, family, and staff on speech-language procedures for carryover.
• Complete compliant CMS documentation in NetHealth EMR systems.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in resident screening to ensure needs are met timely and with clinical excellence.
