Job summary

Evolve Therapy Services in Montgomery, AL is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility on a PRN basis. The role includes planning, directing Speech Language Pathology services, and ensuring CMS-compliant documentation.

Responsibilities include evaluating residents, formulating treatment plans with goals, and coordinating FEES/MBS protocols. Must hold CCC-SLP, active license, and maintain licensure with ongoing CEU activities.

Qualifications

• Master's degree in Speech Language Pathology required.
• CCC-SLP certification required.
• Active SLP license in state(s) of practice required.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target lists and short/long-term goals.
• Instruct patient, family and staff in speech-language procedures for daily carryover.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in resident screening to ensure timely care.
