Job summary

Evolve Therapy Services, LLC is hiring a Speech Language Pathologist for an in-house Skilled Nursing Facility in Montgomery, Alabama. This PRN position focuses on planning, evaluating, and providing comprehensive speech therapy services to residents.

You will develop treatment plans, document per CMS with NetHealth EMR, and coordinate with staff and families to ensure goals are met. Licensure and CCC are required, with ongoing professional development encouraged.

Qualifications

• Master's degree in Speech-Language Pathology required.
• Certificate of Clinical Competency (CCC) required.
• Active SLP license in state(s) of practice.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop a treatment plan with targets, short-term and long-term goals.
• Instruct patient, family, and staff to ensure carryover of therapy goals.
• Complete CMS-compliant documentation in NetHealth EMR.
