Job summary

Evolve Therapy Services is hiring a Speech Language Pathologist for an in-house Skilled Nursing Facility in Burlington, WI. This position offers Full Time, Part Time, or PRN options with competitive pay and CMS-compliant documentation duties.

Responsibilities include delivering quality therapy, data collection for treatment planning, creating goals, training residents and staff, and adherence to FEES/MBS protocols using NetHealth EMR. Active SLP licensure is required, with CCC preferred.

Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency (CCC)
• Must have active SLP license in state(s) of practice
• Ongoing continuing education and professional development to maintain licensure and certification

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• Evaluate and obtain data necessary for treatment planning and implementation
• Develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• Complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence
