Evolve Therapy Services is hiring a Speech Language Pathologist for an in-house Skilled Nursing Facility located in Annandale, VA. The position offers FT/PT/PRN options to provide comprehensive speech-language pathology services within a facility setting.

Responsibilities include developing treatment plans, data-based therapy, staff instruction, and ensuring CMS-compliant documentation using NetHealth EMR. Licensing, CCC, and ongoing CE are required to maintain licensure.

Qualifications

• Master's degree in Speech-Language Pathology required.
• CCC (Certificate of Clinical Competency) required.
• Active SLP license in applicable state(s).
• Commitment to ongoing licensure and professional development.

Responsabilités

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target lists and goals.
• Instruct patients, families, and staff to carry over therapy goals.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversight of FEES/MBS protocols and site coordination.
• Participate in screening protocols to identify resident needs.
