Job summary

Evolve Therapy Services in Madisonville, TN seeks a compassionate Speech Language Pathologist for an in-house Skilled Nursing Facility. This part-time or PRN position offers the chance to deliver high-quality therapy tailored to residents' needs.

Qualifications

• Master's degree in Speech Language Pathology required.
• CCC and state licensure required/active.
• Commitment to ongoing professional development.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate data for treatment planning and progress monitoring.
• Develop and implement individualized treatment plans with short and long-term goals.
• Educate residents, families, and staff to promote carryover of therapy.
• Complete CMS-compliant documentation in NetHealth EMR.
• Coordinate FEES/MBS protocols and site program oversight.
• Contribute to screening protocols to identify residents' needs in a timely manner.

Education

Master's degree in Speech Language Pathology

Certificate of Clinical Competency

Active SLP license in state(s) of practice

Continuing education to maintain licensure
