Job summary

Evolve Therapy Services in Wisconsin seeks a Speech Language Pathologist to join our in-house team at a Skilled Nursing Facility in Suring. You will provide quality speech therapy services, evaluate data for treatment planning, and develop short- and long-term goals.

Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency (CCC)
• Active SLP license in state(s) of practice
• Ongoing continuing education and professional development to maintain licensure

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data necessary for treatment planning.
• Develop treatment plans with target lists and short/long-term goals.
• Instruct patients, families, and staff in speech-language procedures for carryover into daily activities.
• Complete compliant CMS documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening protocols to identify resident needs in a timely manner.
