Job summary

Evolve Therapy Services is a therapy management organization hiring compassionate therapists for our partner located in Paris, TN!

We seek a Speech Language Pathologist for an in-house Skilled Nursing Facility. This PRN role requires a Master’s in Speech Language Pathology, a CCC, active SLP licensure, and ongoing professional development to maintain licensure. Growth opportunities are available based on need and performance.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan with target list, short-term and long-term goals.
• Instruct patient, family, and staff in appropriate speech-language procedures to carry over remediation and goals.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening protocols to determine residents' needs.

Education

Master's degree in Speech Language Pathology

Certificate of Clinical Competency

Active SLP license in state(s) of practice

Ongoing continuing education to maintain licensure and certification
