Job summary

Evolve Therapy Services, LLC in Tiffin, Ohio is seeking a compassionate Speech Language Pathologist for an in-house Skilled Nursing Facility. This position is available as Full Time or PRN and focuses on planning, directing, and delivering speech-language pathology services to residents, while ensuring CMS compliance and high quality patient care.

You will assess residents, develop measurable treatment plans, educate patients and staff, and document in NetHealth.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency (CCC-SLP).
• Active SLP license in state(s) of practice.
• Ongoing continuing education and professional development.

Responsibilities

• Provide quality speech therapy services and initiate referrals per standards.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plan with target list, short-term and long-term goals.
• Instruct patient, family, and staff to carry over therapy goals in daily activities.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversight and implementation of FEES/MBS protocols and site coordination.
• Screen residents to ensure needs are met timely and with clinical excellence.

Education

Master's degree in Speech Language Pathology

Certificate of Clinical Competency

Active SLP license in state(s) of practice
