Job summary

Evolve Therapy Services, LLC is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in Gatesville, NC. This position supports full time, part time, or PRN arrangements and requires a current SLP license in the state(s) of practice.

The role emphasizes evaluating residents, developing treatment plans, and ensuring CMS-compliant documentation in the NetHealth EMR system, with opportunities for growth and competitive pay.

Qualifications

• Master's degree in Speech Language Pathology is required.
• Certificate of Clinical Competency is required.
• Must have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan with target list, short-term and long-term goals.
• Instruct patient, family, and staff in speech-language procedures for carry-over.
• Complete compliant and medically necessary documentation as per CMS expectations in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening protocols to ensure resident needs are met timely and with clinical excellence.

Education
