Job summary

Paradigm at Woodwind Lakes in Houston, TX is seeking a Full-Time Physical Therapist to join our in-house rehab team. You will evaluate residents, tailor personalized treatment plans, and deliver evidence-based care to promote mobility and independence.

New grads are encouraged to apply, with mentorship and a collaborative, resident-centered environment. You'll work with nursing and rehab colleagues to maintain high standards of care, ensure accurate documentation, and participate in IDT

Qualifications

• Active Texas Physical Therapist license in good standing.
• SNF or long-term-care experience preferred.
• Familiarity with Net Health documentation system is a plus.
• Strong communication and interpersonal skills.
• Ability to work effectively with diverse adult and geriatric populations.

Responsibilities

• Perform comprehensive evaluations of residents' physical conditions and functional abilities.
• Develop and implement customized treatment plans using evidence-based practices.
• Provide therapeutic interventions to improve mobility, balance, strength, and independence.
• Monitor progress and adjust care plans in collaboration with the IDT.
• Educate residents, caregivers, and families on mobility techniques and home exercises.
• Ensure timely and accurate documentation in line with facility and regulatory standards.
• Participate in team meetings and support an integrated care approach.
• Adhere to all safety, compliance, and regulatory requirements.
