Job summary

AristaCare Health Services is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility in New Jersey. This full-time role supports resident care and therapy program implementation.

The PTA will perform evaluation-assisted treatment planning, ongoing screenings, and documentation in NetHealth EMR, ensuring CMS-compliant records and coordinated care with the interdisciplinary team.

Qualifications

• Minimum of Associate's Degree in Physical Therapy.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy assistant services to residents per established standards of practice and departmental procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct regular resident screenings to determine intervention needs.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals and methods to achieve them.
• Communicate evaluation findings and plans to residents and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
