Job summary

Liberty Healthcare and Rehabilitation Services is seeking a Physical Therapist (PT) for Southwood Nursing & Rehabilitation Center in Clinton, NC. You will work with a multidisciplinary team to develop individualized treatment plans and provide in-house rehab services with progressive modalities for a variety of conditions.

Qualifications include graduation from an accredited therapy program and state licensure. Prior SNF rehab experience is preferred.

Qualifications

• Graduate of an accredited therapy program.
• Must be licensed in the state of the SNF you will be working.
• Previous rehab SNF experience preferred.

Responsibilities

• Collaborate with a multidisciplinary team to create and implement individualized treatment plans.
• Provide in-house therapy with progressive modalities addressing a range of diseases and impairments.
• Support families through the rehabilitation process and ensure a quality rehab experience.
