Job summary

Claremont Nursing & Rehabilitation Center in Carlisle, PA is seeking a dedicated Full-Time Physical Therapist (PT) to join our in-house rehabilitation team. You will evaluate residents, develop individualized plans, and provide skilled services to improve mobility and independence.

Qualifications

• Current Pennsylvania Physical Therapist license or eligibility for licensure.
• Strong clinical, communication, and documentation skills.
• Commitment to providing high-quality, patient-centered care.
• SNF/LTC experience preferred, but not required.

Responsibilities

• Evaluate residents and develop individualized treatment plans
• Provide skilled physical therapy services to improve mobility, strength, balance, endurance, and functional independence
• Collaborate with physicians, nursing staff, and rehabilitation team members
• Complete timely and accurate documentation
• Ensure compliance with facility, state, and federal regulations
