Job summary

Evolve Therapy Services in Milwaukee, WI seeks a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility. The role plans, organizes, and directs Occupational Therapy services to ensure high quality resident care in compliance with federal, state, and local standards.

The OTA will screen residents, implement treatment plans under supervision, document findings, coordinate with the interdisciplinary team, and schedule therapy sessions.

Qualifications

• Associate's degree in Occupational Therapy.
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification

Responsibilities

• Screen residents to determine best intervention or treatment based on scope of practice.
• Implement treatment plans and follow short- and long-term goals.
• Interpret and communicate evaluation findings to residents, families, and interdisciplinary team.
• Establish individual resident therapy schedules with team members.
• Document and bill occupational therapy services as per treatment plan.
• Perform other duties to effectively treat patients; comply with CMS documentation standards.

Education
