Job summary

Evolve Therapy Services, LLC in Rochester, MN is seeking a licensed Occupational Therapist to provide in-house SNF therapy on a PRN basis. You will screen residents, assess needs, and develop individualized treatment plans within a collaborative interdisciplinary team.

Must hold an active Occupational Therapy license in Minnesota, maintain ongoing CE, and participate in treatment documentation and interprofessional communication. Supervise COTA as needed while delivering quality resident care.

Qualifications

• Degree in Occupational Therapy.
• MUST have an Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain current licensure and certification

Responsibilities

• Conduct regular screening of residents to determine intervention or treatment for each patient.
• Evaluate residents to inform and develop treatment plans; set short- and long-term goals.
• Interpret and communicate findings, plans, and recommendations to residents, families, and team members.
• Establish individual resident therapy schedules in cooperation with team members.
• Conduct therapy per treatment plan and ensure proper documentation and billing.
• Perform other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments meet reimbursement practices.

Education

Degree in Occupational Therapy

Active OT license in practicing state(s)
