Job summary

Evolve Therapy Services, LLC is seeking a compassionate Occupational Therapist for an in-house Skilled Nursing Facility in Hopkins, MN. The OT will plan, organize, develop, and direct therapy services in accordance with federal, state, and local standards, ensuring high quality resident care.

Responsibilities include screening residents, evaluating and planning treatment, coordinating with the interdisciplinary team, scheduling therapy, documenting and billing, supervising COTA, and maintaining

Qualifications

• Must hold a degree in Occupational Therapy.
• Active OT license in the practicing state(s).
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Screen residents to determine the best intervention for each patient.
• Evaluate residents to plan goals and methods to achieve them.
• Explain evaluation findings, plans, and recommendations to residents, families/caregivers, and the team.
• Establish individual therapy schedules in collaboration with other team members.
• Deliver therapy according to the treatment plan and manage documentation and billing.
• Perform all other duties as required to effectively treat patients.
• Oversee COTA and ensure documentation and treatments meet clinical and reimbursement standards.

Job Position

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
