Job summary

Cypress Creek Rehabilitation & Healthcare Center in Cypress, TX, is seeking a Full-Time Occupational Therapist (OT) to join our in-house rehabilitation team. You will assess residents, develop treatment plans, and collaborate with the interdisciplinary team to improve function and independence.

Qualifications

• Active Texas Occupational Therapist license in good standing.
• SNF or long-term care experience preferred.
• Familiarity with Net Health is a plus.
• Strong communication and interpersonal skills.

Responsibilities

• Evaluate residents to determine functional impairments and therapy needs.
• Develop and implement individualized treatment plans focusing on ADLs and IADLs.
• Track progress and adjust treatment plans in coordination with the IDT.
• Recommend adaptive equipment and home modifications to support independence.
• Educate patients, caregivers, and families on therapeutic strategies.
• Ensure accurate, timely documentation per state, federal, and facility guidelines.
• Participate in team meetings and care planning activities.
• Adhere to all regulatory and facility policies.
