Job summary

Mission Park Healthcare Center in Santa Barbara, CA is actively seeking a full-time and PRN Occupational Therapist to join our in-house rehab team within a skilled nursing and rehabilitation facility. Responsibilities

• Evaluate patients in timely manner following physician referral
• Develop effective treatment plans and provide education for client, family and caregivers as subject matter expert
• Communicate with supervisor and other health team members regarding patient progress, problems and plans
• Participate in inservice training program for other staff in the facility
• Document all regulatory and clinically required information in the electronic documentation system and maintain patient records according to company policy
• Provide accurate and timely billing records and supporting clinical documentation
• Instruct patientamilies or nursing staff in maintenance programs
• Coordinate necessary durable medical equipment and adaptive equipment for patients to facilitate safety and independence in ADL's
• Assure resident abuse or neglect does not occur
• Observe confidentiality and safeguards for all patient information
• Complies with federal, state, and local legal and professional requirements
