Job summary

Evolve Therapy Services, LLC is seeking a compassionate Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Selma, AL.

The role involves planning and directing OT services, conducting screenings, implementing treatment plans, documenting care and billing, and collaborating with an interdisciplinary team. A OTA license and an Associate degree are required.

Full-time or PRN opportunities with benefits and growth within our therapy management network.

Qualifications

• Associate's degree in Occupational Therapy.
• Must have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention and treatment.
• Perform treatment plans and monitor short- and long-term goals.
• Interpret and communicate evaluation findings to residents, families, and the team.
• Establish individual resident therapy schedules with team members.
• Conduct therapy per plan and document/bill OT services.
• Perform other duties to effectively treat patients.
• Comply with CMS documentation standards daily.
