Job summary

Evolve Therapy Services is hiring a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility located in Central City, Nebraska. You will work with an interdisciplinary team to provide compassionate OT services to residents.

Responsibilities include conducting screenings, implementing treatment plans under supervision, documenting care, and ensuring CMS documentation standards are met.

Qualifications

• Must have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification

Responsibilities

• Screen regular residents to determine the best intervention or treatment based on scope of practice.
• Perform treatment plans; follow short- and long-term goals and methods to achieve goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned to effectively treat patients
• Comply with CMS documentation standards of practice daily with point of care documentation as indicated
