Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Greendale, WI. Positions available: Full Time, Part Time, or PRN.

The OTA will plan, implement, and supervise therapy under the direction of a licensed therapist, ensuring high-quality resident care. Requirements include an Associate's degree in Occupational Therapy and active OTA licensure in state(s) of practice, with ongoing continuing education.

Qualifications

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification

Responsibilities

• Conduct regular screening of residents to determine appropriate intervention or treatment
• Perform treatment plans and follow short- and long-term goals
• Interpret and communicate evaluation findings to residents, families, and interdisciplinary team
• Establish individual resident therapy schedules with team members
• Conduct therapy according to treatment plan and document/bill appropriately
• Perform other duties to effectively treat patients as assigned
• Maintain CMS documentation standards of practice daily with point-of-care documentation
