Evolve Therapy Services is seeking a compassionate Occupational Therapist for an in-house Skilled Nursing Facility in Hopkins, MN. This PRN role requires planning, organizing, and directing OT services in alignment with federal, state, and local standards to ensure high-quality resident care.

Responsibilities include screenings, evaluations, treatment planning, documentation, and collaboration with the interdisciplinary team.

Formación

• Must have an active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure.

Responsabilidades

• Conduct regular screening of residents to determine the best intervention.
• Evaluate residents to inform and develop treatment plans with goals.
• Interpret and communicate evaluation findings and recommendations to residents and families.
• Establish individual resident therapy schedules in cooperation with the team.
• Perform therapy according to treatment plans and document/bill services.
• Oversee COTA as needed and ensure compliance with documentation and reimbursement practices.

Educación

Degree in Occupational Therapy

Active Occupational Therapy license in practicing state(s)

Ongoing continuing education and professional development
