Job summary

Valley West Post Acute in Williams, CA is seeking therapists to join an in-house therapy team. The role involves evaluating patients, creating and implementing intervention plans, and using modalities and equipment to improve function.

You will work within a collaborative IDT, supervise PTAs or rehab aides, and maintain complete patient records while documenting progress and discharge summaries in accordance with regulatory requirements.

Qualifications

• Active State Licensure.

Responsibilities

• Assures efficient management of work processes.
• Provides initial evaluation of patient's abilities and designs appropriate intervention plans.
• Treats patients with therapeutic procedures, modalities and/or equipment to improve function.
• This position reports to the Director of Rehabilitation Services.
• Consult with patients to learn about their physical condition and symptoms; review medical history and referrals.
• Diagnose movement dysfunction by observing patients.
• Develop individualized treatment plans.
• Coach patients and teach therapeutic massage to ease pain and promote healing.
• Maintain patient records and track goals.
• Utilize standardized assessments and evidence-based practice in clinical interventions.
• Assess progress and adjust treatments in the clinical record.
• Evaluate treatment outcomes and modify services accordingly.
• Provide consultation to patients, families, and care providers regarding physical disorders.
• Communicate progress and plans with supervisor and health team members.
• Provide comprehensive discharge summaries per regulatory requirements.
• Document all regulatory and clinical information in the electronic system.
• Participate in trainings, in-services and conferences.
• Able to supervise and support PTAs; rehab aides and/or students.
• Adhere to company policies and code of conduct.
• Active State Licensure.
