Hayward Gardens Post Acute in Hayward, CA is seeking a compassionate Physical Therapist (PT) to join our in-house rehabilitation team. We evaluate residents, create individualized plans, and provide hands-on therapy to help residents regain strength and independence.

Qualifications

• Current and active Physical Therapist (PT) license in California.
• Experience in skilled nursing or geriatric rehabilitation preferred.
• Strong clinical assessment, treatment planning, and communication skills.
• Ability to work collaboratively with an interdisciplinary team.
• Excellent organizational and documentation skills.
• Commitment to providing compassionate, high-quality resident care.

Responsabilités

• Evaluate residents and identify physical therapy needs, functional limitations, and rehabilitation potential.
• Develop and implement individualized treatment plans to improve strength, balance, mobility, endurance, and functional independence.
• Provide skilled physical therapy treatments and monitor progress toward established goals.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Collaborate with physicians, nursing staff, residents, families, and interdisciplinary team members.
• Participate in care conferences, rehabilitation meetings, and discharge planning.
• Educate residents and families on exercise programs, fall prevention, safety techniques, and continued care.
• Recommend appropriate durable medical equipment to support safe mobility and independence.
• Maintain accurate, timely, and compliant clinical documentation.
• Assist with staff education, restorative nursing programs, and quality improvement initiatives.
