Job summary

Diakon Behavioral Health Services in Mechanicsburg, PA, is seeking a full-time, varied-hours counselor and case manager for the Specialized In-Home Treatment program.

You will work with youth and families, provide skill development in home, coordinate services, and participate in court proceedings as needed. A bachelor’s degree in social work or related field and at least one year of relevant experience are required.

Qualifications

• Bachelor’s degree in social work or related field required.
• Minimum of one year experience in family or related child welfare or human service position.
• Ability to sit, stand, bend, stoop, reach and lift up to 50 lbs.

Responsibilities

• Serve as resource person, case manager, counselor and trainer for the Specialized In-Home Treatment program.
• Establish and maintain professional relationships with youth, families, referral personnel and treatment providers.
• Meet regularly in the youth’s home to provide skill development and ongoing case management.
• Maintain regular contact with referring personnel and testify at court proceedings involving children in care.
