Job summary

Pathways Healthcare, a leader in in-home nursing and hospice care in Massachusetts, is hiring a registered nurse to support our Medical Rehabilitation Program. You will assess patients, administer treatments, and develop individualized care plans with physicians and teams.

Qualifications

• Active nursing license in the state where the position is based.
• Active WOCN certification.
• Prefer 2+ years of geriatric nursing experience.
• Nursing degree from an accredited institution.
• Excellent bedside manners and communication skills.
• Knowledge of healthcare laws and regulations.
• Current BLS CPR certification.
• Valid driver's license and reliable vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.
