Job summary

Apex Rehab Group is seeking Occupational Therapists, Physical Therapists, and Speech Therapists to join our home care team. If you enjoy working one-on-one with patients, and having flexibility in your schedule, this could be a great fit.

You will provide therapy services in patients' homes, create personalized treatment plans, track progress, educate families, and work with a clinical team for support. Licensing in New York is required; home care experience is a plus.

Qualifications

• Licensed OT, PT, or ST in New York.
• Home care experience is a plus but not required.
• Reliable transportation and a valid driver’s license.
• Excellent communication and a positive attitude.
• Ability to manage your own schedule.

Responsibilities

• Provide therapy services to patients in their homes.
• Create and carry out personalized treatment plans.
• Track patient progress and adjust care as needed.
• Educate patients and families.
• Complete simple, timely documentation.
• Work independently with support from our clinical team.
