Qualifications

• Good communication skills and a positive attitude.
• Reliable transportation and a valid driver’s license.

Responsibilities

• Provide therapy services to patients in their homes.
• Create and carry out personalized treatment plans.
• Track patient progress and adjust care as needed.
• Educate patients and families.
• Complete simple, timely documentation.
• Work independently with support from our clinical team.
