Job summary

YAI - Seeing beyond disability seeks a Therapeutic Coach for East Syracuse to support individuals with crisis or planned services using person-centered interventions within homes and community settings.

You will conduct coaching visits, provide feedback, and plan activities to build skills for positive supports while collaborating with caregivers and circles of support. A bachelor’s degree and 2+ years of experience are required.

Qualifications

• Bachelor’s degree in psychology, social work or related human services field.
• At least two years of experience with the I/DD population or behavioral health needs.
• Willingness to travel regularly across New York.
• Ability to meet essential physical demands of coaching in home/community settings.

Responsibilities

• Supports individuals across assigned region via crisis or planned services with person-centered interventions.
• Conducts coaching visits and provides feedback on outcomes.
• Plans social and leisure activities to build skills for positive supports.
• Completes documentation and enters data into START Information Reporting System.
• Travels regularly to conduct in-person coaching sessions.
