Job summary

YAI seeks a dedicated Therapeutic Coach to support individuals in home and community settings across assigned regions. You will implement person-centered interventions, de-escalate crises, and work with caregivers and circles of support to promote safety and meaningful activities.

The role requires collaboration with START leadership and clinical teams, travel across regions, and strong documentation practices to ensure progress is tracked and services adjusted as needed.

Qualifications

• Bachelor’s degree in psychology, social work or other human services field required.
• At least two years of experience with I/DD population and/or behavioral health.
• Ability to travel regularly across assigned region.
• Flexible and creative in designing engaging activities.

Responsibilities

• Provide in-home and community-based therapeutic coaching.
• Deliver person-centered interventions and de-escalation strategies.
• Conduct coaching visits and gather progress data.
• Collaborate with START team and caregivers.
• Plan social/leisure activities to build skills.
• Complete documentation and reporting in systems.
• Transport supplies and manage recreational materials.
• Participate in crisis prevention planning.
