Job summary

BAYADA Home Health Care in Rutland, Vermont is seeking an experienced Speech Language Pathologist (SLP) for a full time, part time or per diem opportunity to perform home health visits across Addison County.

As a member of a multidisciplinary team, you will conduct evaluations, develop and adjust treatment plans, educate families, and document care using EMR. VT licensure and CCC-SLP (or equivalent) are required; home health experience is a plus.

Qualifications

• Active VT licensure as Speech Language Pathologist.
• CCC-SLP or equivalent certification in progress or obtained.
• Experience with EMR systems preferred; strong written documentation.

Responsibilities

• Make home visits in designated territory (Rutland and Addison County).
• Perform evaluations and diagnose speech, language, communication or swallowing issues.
• Develop and adjust treatment plans based on evaluations and physician orders.
• Educate patients and families on rehabilitative care and activities.
• Document observations and interventions accurately in EMR.
