Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to deliver one-on-one care to older adults in Jefferson, IA, in private homes or senior living communities. You will assess, plan, and treat within the Geriatric House Calls™ model, building strong patient relationships and managing your own schedule.

This role offers clinical autonomy, collaboration with an interdisciplinary team, and the potential to incorporate telehealth to extend access.

Qualifications

• Must hold a valid Speech Language Pathology license in the state of practice or be eligible to obtain it.
• Degree from an accredited Speech Language Pathology program.
• Current CPR certification.
• Current Certificate of Clinical Competence (ASHA CCC)

Responsibilities

• Provide one-on-one Speech Language Pathology services to older adults in private residences or senior living communities.
• Perform evaluations, develop individualized care plans, and deliver evidence-based treatment.
• Deliver therapy sessions to support patient wellness, mobility, and independence.
• Document effectively within FOX’s EMR system in compliance with regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access and flexibility.
