Job summary

FOX Rehabilitation in Decorah, IA is seeking licensed Speech Language Pathologists to deliver one-on-one care to older adults in private residences or Senior Living Communities through the Geriatric House Calls model.

You will conduct evaluations, develop plans of care, and deliver therapy aimed at improving mobility, independence, and quality of life, with schedules you manage and telehealth options to extend care.

Qualifications

• Licensed Speech-Language Pathologist in the state(s) of practice or eligible to obtain license.
• Master's degree in Speech-Language Pathology from an accredited program.
• Proficiency with basic computer use and EMR systems.
• Current CPR certification and CCC certification (ASHA).

Responsibilities

• Provide one-on-one speech-language pathology services to older adults in homes or senior living communities.
• Conduct evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and functional independence.
• Document using FOX’s EMR system in compliance with regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
