Job summary

CommonSpirit Health is seeking a licensed Speech-Language Pathologist to join the VNA Home Health team. The role involves assessing, diagnosing and treating patients in their homes, developing individualized treatment plans, and collaborating with families and other healthcare professionals to achieve optimal outcomes.

Responsibilities include delivering therapy at patients’ homes, tailoring strategies to goals, documenting progress, and training families to support exercises at home.

Qualifications

• Masters degree in Speech-Language Pathology from an accredited program.
• Active registered SLP license in the state which you practice and CCC-SLP certification from ASHA.
• Must obtain and maintain current CPR/BLS Certification
• One-year Speech Therapy experience in a clinical setting.
• Current Drivers License
• Comply with federal, state and local laws and regulations

Responsibilities

• Provide therapy to patients in their homes, including techniques to improve speech, language, cognitive-communication, and swallowing skills.
• Customize therapy strategies to meet individual patient goals and objectives.
• Maintain accurate and timely documentation of patient progress, treatment goals, and outcomes in accordance with regulatory requirements and standards.
• Provide training and support for family members to assist with therapeutic exercise and strategies at home.
