Job summary

Qualifications

Responsibilities

• Collaborates with the interdisciplinary team to identify patients for Social Work Services.
• Completes comprehensive in-home psychosocial evaluations.
• Assesses cognitive status and behavioral health needs.

• Advocates for patient access to care.
• Maintains timely documentation of social work services.
