Job summary

Fox Rehabilitation in Cypress, TX is seeking a licensed Speech Language Pathologist to provide in-home and senior living care. You will conduct evaluations, create individualized care plans, and deliver evidence-based therapy to older adults in private residences and communities.

The role offers autonomy, flexible scheduling, and opportunities to incorporate telehealth to extend access and continuity of care.

Qualifications

• Active Speech Language Pathology license in the state of practice or eligibility to obtain one.
• Basic computer and EMR proficiency required.
• Current CPR certification required.
• Current CCC (ASHA) certification preferred or required.

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care, deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, longevity, mobility, and independence.
• Document thoroughly in FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
