Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to deliver one-on-one geriatric care in Greeley, CO, including homes and senior living communities. You will assess mobility, design rehab plans, and help patients maintain independence through our Geriatric House Calls model.

The role offers autonomy with flexible scheduling, optional telehealth, and collaboration within an interdisciplinary team. Full-time positions include benefits; part-time and PRN options are also available.

Qualifications

• Active Physical Therapy license in the state(s) of practice.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Deliver therapy sessions to support mobility, independence, and functional outcomes.
• Document therapy in FOX's EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PT, OT, SLP, fitness specialists).
• Offer telehealth/virtual care when appropriate to extend access.
