FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in private residences and senior living communities across Colorado. You’ll build patient relationships, manage your own treatment schedule, and deliver high-impact outcomes through our geriatric house calls model.

Formación

• Valid Physical Therapy license in the state of practice.
• DPT from an accredited PT program.
• Proficiency with EMR systems and basic computer skills.
• Current CPR certification.

Responsabilidades

• Provide one-on-one physical therapy to older adults in home or community settings.
• Perform evaluations and develop individualized plans of care.
• Deliver evidence-based treatment to improve mobility and independence.
• Document in FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety and aging in place.
• Provide telehealth when appropriate to extend access.
