Top Echelon Software LLC in Casper, WY is seeking a full-time Registered Nurse (RN) to join our home health team. You will provide high-quality patient care to seniors in their homes, working with an interdisciplinary team to develop and implement care plans.

The role requires a Wyoming RN license, flexibility for evenings/weekends/holidays, and reliable transportation with mileage reimbursement. Per diem shifts may be available as needed.

Qualifications

• Current RN license in Wyoming.
• 0+ years of home health experience.
• Ability to work flexible hours including evenings, weekends, and holidays.
• Valid driver's license and reliable transportation (mileage reimbursed).

Responsabilités

• Provide skilled nursing care to seniors in their homes, assessing their physical, emotional, and social needs.
• Develop and implement individualized care plans in collaboration with patients, families, and interdisciplinary team members.
• Administer medications, treatments, and therapies as prescribed by physicians, and monitor patient responses.
• Educate patients and families on disease management, self-care, and wellness strategies.
• Collaborate with other healthcare professionals to ensure comprehensive care and coordination of services.
• Maintain accurate and detailed records of patient interactions, assessments, and care plans.
