Qualifications

• Current and unrestricted RN license in Wisconsin.
• Current BLS/CPR certification.
• Current TB test or Chest X-Ray.
• Nursing experience preferred (1 year+).
• Strong clinical, communication and documentation skills.

Responsibilities

• Deliver skilled nursing care per physician orders and care plans.
• Assess, monitor, and document patients' health status.
• Identify changes in patient conditions and communicate findings.
• Develop and implement individualized care interventions.
• Collaborate with patients, families and healthcare teams to ensure quality care.
• Maintain accurate and timely clinical documentation.
