Job summary

Qualifications
- Current and unrestricted RN license in Wisconsin.
- Current BLS/CPR certification.
- Current TB test or Chest X‑Ray.
- Previous nursing experience preferred (1 year or more).
- Strong clinical, communication, and documentation skills.
- Commitment to providing compassionate, patient-centered care.
Responsibilities
- Deliver skilled nursing care in accordance with physician orders and care plans.
- Assess, monitor, and document patients' physical and emotional health status.
- Identify changes in patient conditions and communicate findings appropriately.
- Develop and implement individualized care interventions.
- Collaborate with patients, families, and healthcare teams to ensure quality care.
- Maintain accurate and timely clinical documentation.
