Job summary

Interim HealthCare is seeking LPN/RN to deliver in-home direct patient care under a physician's plan of care, closely monitoring progress and documenting findings. The role requires a current RN/LPN license in the state, CPR/BLS certification, and reliable transportation to care sites.

You will administer medications, manage IV/fluids, assess wounds, and collaborate with a care team to ensure optimal patient outcomes.

Qualifications

• Graduate of a practical nursing program and a current RN/LPN/LVN license in the applicable state.
• CPR/AED/BLS/First Aid certification required.
• Reliable transportation to/from care sites and/or work locations.
• Trach and/or ventilator experience preferred, not required.

Responsibilities

• Provide in-home direct patient care according to the plan of care and nursing scope of practice.
• Assess and document patient conditions and report changes (vitals, labs, urine, etc.).
• Administer medications, insulin, IV/fluids, and document thoroughly.
• Inspect and care for wounds, dressings, and personal hygiene.
• Assess for further injuries or infections and provide appropriate treatments.
• Collaborate with providers and other health professionals to ensure patient outcomes.
