Job summary

Giving Home Health Care is seeking a compassionate RN or LPN for the Southern Illinois area including Metropolis to administer patient care at patients' homes per physician plans. This is a 1099 contract with flexible shifts, ranging from weekly visits to full 12-hour shifts or a blend of both.

Shifts include day, evening, night and weekends; sign-on bonus of $5,000 for 24‑hour homes. Background checks are required and travel to patient homes is expected.

Qualifications

• Graduate of an accredited college or university (Associate or Baccalaureate School of Nursing).
• Current State or Compact RN or LPN license.
• Current CPR certification.
• TB test and physical examination.
• Maintain required insurance.
• Must have dependable transportation and be willing to commute to patients' homes.
• Ability to provide basic medical management up to medically fragile end of life care.

Responsibilities

• Execute physician prescribed plans of care.
• Complete compliant documentation of the care provided in the system of record.
• Manage and administer prescribed medication, treatment, and therapies.
• Conduct patient assessments and coordination of care.
• Perform activities of daily living as needed.
• Monitor vitals and intake/output.
• Assess skin integrity and administer wound care.
• Communicate with family, physicians, and case managers about patient health.
• Provide education and training to patient/family/Home Health Aide.
