Job summary

Giving Home Health Care is seeking a compassionate RN or LPN to provide one-on-one nursing care in the patient’s home in Monument Valley, AZ. The role operates as a 1099 independent contractor with a schedule of 10 hours per week, following a physician-established plan of care.

The position offers a $2,500 sign-on bonus, paid in two installments, and collaboration with CNAs, HHAs, and case managers to ensure coordinated, quality care. Arizona licensure and CPR are required.

Qualifications

• Graduate of an accredited college or university with nursing licensure.
• Current State or Compact RN or LPN license for AZ.
• CPR certification.
• Ability to lift up to 50 lbs and move patients.

Responsibilities

• Provide skilled nursing care in the patient’s home per physician plan of care.
• Develop a professional relationship with the patient and act as their advocate.
• Monitor patient condition and communicate changes to the care team.
• Administer medications and treatments within scope of practice.
• Coordinate care with CNAs, HHAs, Case Managers, physicians, and other providers.
• Maintain accurate clinical documentation and ensure patient safety.
