Job summary

UPMC Home Health is seeking a full-time RN to serve McKnight, Allison Park, Glenshaw and surrounding areas. The role offers a ladder of growth from Professional Staff Nurse to Expert Nurse, with opportunities for schedule input and one-on-one patient care in home settings.

Responsibilities include planning patient care with physicians and UM staff, coordinating multi-disciplinary case management, and delivering patient and family education.

Qualifications

• Active RN license in state or multistate compact.
• BSN preferred.
• Six months to two years of nursing experience preferred.

Responsibilities

• Establish patients' plans of care in collaboration with physicians, patients and UM professionals to meet orders and patient needs.
• Coordinate case management activities across a multidisciplinary team to ensure quality patient care.
• Initiate, deliver, and evaluate patient- and family-centered health teaching.
• Contribute to achieving optimal health outcomes with compassionate, patient-focused care.
• Complete all required documentation (visit assessments, OASIS, 485, verbal orders) accurately in home setting.
