Job summary

Interim HealthCare Inc. in West Virginia is seeking a Registered Nurse (RN) to provide in-home direct patient care under the prescribed plan of care.

You will monitor progress, document vitals, and collaborate with the care team to deliver compassionate, high-quality service in clients' homes. Eligible candidates hold an Associate degree in Nursing (AND or AAS), an unrestricted RN license in WV, CPR/AED/BLS certification, and reliable transportation.

Qualifications

• Associate degree in Nursing (AND) or AAS required
• Current unrestricted RN license to practice in West Virginia
• Current CPR/AED/BLS/First Aid certification
• Reliable transportation to/from care sites and/or work locations

Responsibilities

• Provide in-home direct patient care according to the provider's prescribed plan of care while closely monitoring progress/recovery
• Assess and document patient conditions and report changes as needed (vital signs, blood/stool/urine, glucose, etc.)
• Administer medication, insulin, and IV/fluids; documenting thoroughly
• Inspect and care for wounds, changing dressings and assisting with personal hygiene
• Assess patient for further injuries, infections, bedsores, etc. and deliver appropriate treatments
• Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes

Education

Associate degree in Nursing (AND) or Associate of Applied Science in Nursing (AAS)
