Job summary

Professional Case Management is seeking a registered nurse (RN) to provide in-home health care in Tennessee. You will build one-on-one relationships with patients, assess needs, administer nursing care, and help maintain comfort and safety in the patient's home.

You can set your own schedule within the in-home care model, with 12-hour night shifts available on Fridays and Saturdays. We offer competitive hourly rates, medical/dental/vision benefits, and Health Savings/ Flexible Spending Accounts.

Qualifications

• Graduate of a state-approved school of professional nursing.
• Current, unrestricted RN license in the state of practice.
• CPR certification current.
• TB test current.

Responsibilities

• Provide in-home nursing care to patients.
• Develop one-on-one relationships with patients and families.
• Assess patient needs and coordinate with care teams.
• Maintain safety and compliance with agency policies.
